Case & Care Management

Our team work guide our services based on our clients needs. We will be responsible for our collaborative process of assessing, planning, facilitating, evaluating, referring, care coordinating, and advocating for options and services to meet an individual and families comprehensive health needs. We will do so by providing clear communication and available resources to promote client safety, quality of care, and cost. In addition, depending on the services needed, we will incorporate client-and-family centered, team-based activities that are designed to assess and meet the clients needs, while helping them navigate effectively and efficiently through the health care system, as well as our program.
